Why is this Plan Important for NHL Alumni and Family members?

Congratulations for enrolling in the Equitable Life Health and Dental Plan for NHLAA and their families. This plan will provide you with enhanced Dental coverage as well as coverage for prescription drugs, vision, travel, paramedical services and more. To review your coverage in more detail please return to your link in the locker room or the Equitable Life booklet that was provided to you.

Step-By-Step Instructions

STEP 1: Download the App and Activate your Account

To download the EZClaim App on your iPhone click here.

To download the EZClaim App on your Android click here.


Once you have downloaded the EZClaim App, open it and look for the link where it says “Activate Account” and click on it.
STEP 2: Choose Plan Member

Then click on the box for “Plan Member“, see below.
STEP 3: Complete Your Personal Information

Fill in your legal name, policy number (816053) and your certificate number, included on your membership card as well as your welcome email.

Congratulations!
You are now able to Submit Claims!
In the App, you will enter your banking information to get paid on claims submitted.
