NHLAA Canadian Health & Dental Plan - Optional Family Coverage
As a Canadian NHL Alumni with 400+ games credited, we are pleased to provide you with this extensive Health & Dental Program. You also have a unique opportunity to extend this same high quality coverage to your eligible dependents, including your spouse and eligible children. This extension allows your family to benefit from a plan that was specifically designed for NHL Alumni and their families – without the traditional barriers and limitations that exist with most other plans.
In addition to the enhanced Dental Coverage, the plan also includes coverage for prescription drugs, vision, travel, paramedical services, and more. This plan is designed to adapt as your family’s needs change over time. Because enrolment is available only during specific periods*, it is important to review your options early. SC Insurance advisors are available to help you assess this coverage alongside existing plans and determine what makes the most sense for you and your family.
A summary of coverage highlights is provided below. All benefits are subject to policy terms, conditions, limitations, and exclusions. For full details, contact SC Insurance or the NHLAA. A copy of the Equitable Life Booklet is below.
